Social Security Number Missing
When entering an application in the Eligibility Specialists Portal you may come across a participant that does not provide a social security number for themselves or another household member. If citizenship is not in question you can enter the evidence in to MEDES using the steps below. These directions MUST be followed precisely as they are written to ensure the most appropriate entry of evidence into MEDES.
You should attempt to locate the SSN using any available sources (i.e. FAMIS, MODL) prior to using this work guide.

TO ENTER AN INCOMPLETE OR MISSING SOCIAL SECURITY NUMBER
- Navigate to the New Application Form entry > More About You screen > Additional Information section > Does this person have an SSN? field
- Select No from the drop down menu
- In the Has this person applied for SSN? field> select Yes from the drop down menu<
- Complete remaining fields
- Click Next
- Complete the application flow and Submit.
- Enter a separated comment on the HoH Person page to read as follows:
- Subject – Social Security Number Unavailable
- Body – Participant failed to provide a social security number for ___ (list household members without SSN) on the application. Attempted to obtain the SSN using all available electronic sources. Selected NO for Does this person have an SSN? field. Selected YES for Has this person applied for SSN? field.
Click Save & Exit periodically to prevent loss of data in the event the application times out or closes unexpectedly during application entry.
