Add Social Security Number to Person Page

When the participant has a Social Security Number (SSN) but it is not captured on their Person Page, follow the directions below to add the SSN.

TO ADD A SSN TO A PERSON PAGE IN MEDES:

1. From the Person Page > Evidence sub-tab > Evidence folder

NOTE: Ensure there are no Evidence Type Identifications with a Description of Social Security Number showing for the participant.

2. Select the New hyperlink. This opens the New Evidence popup box.
3. Select the List Action menu next to the Identifications Type row.
4. Select Add
5. In the Received Date field, select the date the SSN is being added.
6. Select the dropdown in the Case Participant field and select the participant the SSN is being added to.
7. In the ID Reference field, type the SSN.
8. Select the dropdown in the Type field and select Social Security Number.
9. In the From date field, ensure the date matches the date in the Received Date field.
10. Select Save
