Home and Community Based Services (HCB) Quick Reference Guide
Home and Community Based Services (HCB) waiver is part of the Missouri’s nursing home diversion program. Through this program, individuals are offered in-home services that may allow them to remain in their own homes, rather than going to a care facility. Individuals must meet certain criteria and be determined medically eligible by the Department of Health and Senior Services, Division of Senior and Disability Services (DHSS/DSDS)
TO REFER AN HCB REQUEST
To complete an HCB Referral, go to IM Resources > Handoffs > Non-MAGI Specialized Units > HCB Hand Off

- Make clear concise notes on EUMEMROL and in the Add Notes field when adding the task.
- Subject line = “Request for HCB
- Body of email
- Customer’s Name
- Customer’s DCN (if applicable)
- Customer’s DOB or SSN
- Phone number where customer (or Authorized Representative), may be reached.
- If customers have additional questions which frontline staff cannot answer, the HCB PC may be reached at 877-304-7939.
- Make clear concise notes on EUMEMROL and in the Add Notes field when adding the task.
HCB PC staff completes the IM-54A and sends it to DSDS.
Do not send an IM-54A to DSDS, when the individual does not meet the HCB eligibility factors. Do not refer individuals who:

- Are under age 65, or under age 63 when disabled.
- Has no need of HCB services
- Has income which exceeds the HCB income limit
NOTE: There is another version of HCB services available directly through DHSS/DSDS, they refer to it as “HCBS”. Basic eligibility requirements include being 18 or older and being or potentially being Medicaid Non-Spenddown. These clients can be referred directly by phone.
