Printing and Preparing FAMIS Documents
When an application or case action is pending in FAMIS for information and verification, generate, edit and print a Request for Information (FA325) which lists the information and verification needed to make a determination.
Generate, edit and print other verification request forms, such as the Authorization for Release of Information (FA106), to help participants obtain the necessary verification for FSD to process applications and case actions timely.
Customer Service Center staff generate and edit FA325s and FA106s. They may print and generate other request forms to print locally or to send to the Outbound Mail Group or Resource Center staff.
Record which document forms were mailed or given to the participant in a summary comment on Eligibility Unit Member Role (EUMEMROL/FM3Z).
Not all forms are generated and printed in FAMIS. Follow local office procedures for creating, editing and printing those forms.
INSTRUCTIONS:
- From DOCQUE, select the forms to print by typing a P on the Com line
NOTE: Never let an unedited FA-325 batch print to be mailed out overnight
- Press F23=PRINT
- Complete and/or highlight fields to help the person providing the information know what is needed.
