Authorized Representative for MO HealthNet

A claimant may designate an Authorized Representative (AR) at any time.

An Authorized Representative can:

    • Help with MO HealthNet application
    • Acting ongoing for annual reviews, reporting changes, etc
    • Or both

The Authorized Representative must complete the application within 30 days of the date the claimant signs the IM-6AR. If the Authorized Representative is a non-relative, the IM-6AR form must be signed prior to the Authorized Representative signing the Application Request. Correspondence to the Authorized Representative is sent to the address on the Representative Detail screen.

When an IM-6AR (or comparable document) is needed:

  • Guardian – No IM-6AR needed if legal paperwork is provided.
  • Conservator – No IM-6AR needed if legal paperwork is provided.
  • Relative – IM-6AR is needed for the relative to help with the application or act ongoing for annual reviews, reporting changes, etc.
  • Non-Relative – IM-6AR is needed.
  • Public Administrator (PA) and his/her deputy – No IM-6AR needed but it must be confirmed the applicant or recipient has been assigned a PA.
  • Power of Attorney– No IM-6AR needed if legal paperwork is provided and it confirms the Power of Attorney has financial, business or legal authority.

If the claimant is deceased, the Application Request must be signed by a court appointed personal representative, administrator, or executor of the deceased applicant’s estate, no later than the third month following the month of death.

If an authorized representative was designated before death, the authorized representative can sign the Application Request.

 

DEFINITIONS:

  • Guardian: a person who has been appointed by a judge to take care of a minor child or incompetent adult (both called “ward”) personally and/or manage that person’s affairs.
  • Conservator: a guardian and protector appointed by a judge to protect and manage the financial affairs and/or the person’s daily life due to physical or mental limitations or old age.
  • Public Administrator: a county official with responsibility to handle the affairs of someone who has died with no known or available relative, executor or friend. At times the public administrator may be instructed by a court to assume similar duties for a living person when no conservator or guardian is available. A Public Administrator is coded LGN or CON in FAMIS.
  • Power of Attorney: also known as “Attorney in Fact”. It is important to read all legal documentation because not all POAs have medical authority. A Power of Attorney is coded ARP in FAMIS.
