Cooperation in Pursuit of Medical Support
The parent or caretaker must cooperate with the Family Support Division-Child Support (FSD-CS) in the pursuit of medical support. A parent or caretaker must complete a CS-201 Referral/Information for Child Support Services form for the absent parent(s).
If there is already a CS-201 Referral/Information for Child Support Services form on file that lists the same children as currently within the household and the children remain with the same custodial parent as listed on the CS-201 Referral/Information for Child Support Services form, a new CS-201 Referral/Information for Child Support Services form does not need to be submitted to Child Support Enforcement.
DO NOT hold an application pending completion of the CS-201.

TO GENERATE A 201 ON AN APPLICATION CASE

- Navigate to the Application Case > Evidence sub-tab > Verifications folder
NOTE: Ensure you have verified all evidence you have verification for before generating the CS-201 Referral/Information for Child Support Services and the accompanying IM-31A Request for Information forms.

- Click the Send notice for outstanding…Wizard
- In the Select The Required Notices popup box, check the following:
- IM-31A Request for information Free Form Text
- CS-201 Child Support Referral Form
- Any of the other listed forms needed
- Click Next
- In the IM-31A Free Form Text popup box, type any information you wish to give to the participant
- Click Next
- In the CS-201 Number of copies needed popup box type the number of CS-201 forms needed in the Number of copies needed field
- Click Next
NOTE: Depending on the additional notices generated (IM-39A, IM-6, IM-60MC) you will need to complete the popup boxes adding information needed

- Click OK in the Notice Sent Verification popup box
- Make a comment on the HoH Person Page to include:
- The CS-201 has been sent
- Review Notice Queue to make sure appropriate forms generated:
- IM-31A Request for Information
- CS-201 Child Support Referral Form
- When the completed CS-201 Child Support Referral Form is received, scan the form(s) to:
- FSD.IntakeCenter@dss.mo.gov when all parties can be identified (custodial, non-custodial/alleged parent and children)
- CS.MEDESLiaison@dss.mo.gov when the non-custodial/alleged parent is unknown
- Scan the CS-201 Child Support Referral Form to the Virtual File Room
- Make a comment on the HoH Person Page including:
- The information contained in this note; the Absent Parent’s Name, Address and Date of Birth from the completed CS-201 form returned by the participant.
- Child of the absent parent(s)
To Generate the CS-201 Referral/Information for Child Support Services on an Integrated Case

- From the Integrated Case > Evidence sub-tab > Verifications folder
- Click the List Action … Menu
- In the Select The Required Notices popup box, check the following:
- IM-31A Request for information Free Form Text
- CS-201 Child Support Referral Form
- Any of the other listed forms needed
- Click Next
- In the IM-31A Free Form Text popup box, type any information you wish to give to the participant
- Click Next
- In the CS-201 Number of copies needed popup box type the number of CS-201 forms needed in the Number of copies needed field
- Click Next
