View Evidence for an Application Case
The evidence required for eligibility is pre-defined in MEDES. Required evidence is associated with each participant on the case and to the case overall. An Eligibility Specialist adds the required verifications to the evidence by electronic sources or by the applicant.
A task is created to verify evidence.
TO VIEW ITEMS TO VERIFY ON AN APPLICATION CASE
NOTE: Before processing the Application Case, review the MAGI Application Processing Guide (Flexibilities) document. If the Application Case fits any of the 3 flexibilities; MAGI Applications Received as FFM Referrals, Enrolling Parents into Medicaid Based on Children’s Income Eligibility or Targeted Enrollment SNAP Strategy, follow guidance for processing the Application Case.

- Navigate to the Application Case home page > Items to Verify hyperlink
- Click the Verify…Wizard
- Select Item used as verification from the drop down menu
- In the Provided By section
- From the Case Participant drop down menu
- Select the HoH for any evidence verified using self-attestation or provided by the participant
- Select the blank line for evidence verified using external sources or other hard copy verification (e.g., IMES, IIVE, SUPD, Pay Stubs, etc.)
- From the Participant drop down menu, select the appropriate source
- In the Name free form field type the external source used (e.g., IMES, IIVE, SUPD, Employers Name, etc)
- From the Case Participant drop down menu
- Click Next
- Check the box for any evidence that can be verified using the Item selected on the Add Proof page. If all evidence can be verified using this Item, check the box on the ribbon bar next to Evidence
- Type Comments as needed MEDES Notes and Comments
- Click Save
- Repeat steps 4-8 until all evidence needing verification is completed
- The verified evidence is removed from the Outstanding Verifications list. The Items to Verify count reduces when the page is refreshed by hitting F5
- Record necessary comments on verified evidence as appropriate MEDES Notes and Comments
- From the Evidence folder > Click the List Actions
menu for desired evidence record
NOTE: You can sort the pieces of evidence by clicking Type in the yellow ribbon bar

- Click Edit. Enter comment on the piece of evidence included but not limited to:
- Income
- Deductions
- Authorized Representative
- Non-Citizens
- Students
- If requesting further verification of a particular piece of evidence because something is in question, a note would be needed on that piece of evidence to explain that verification has been requested.
