Address Change on an Application Case
When a participant reports a Change Of Address you must add the new home and mailing address in MEDES. If only one or the other has changed, only add that new address type.
NOTE: Even if the participant has moved out of state, you MUST still add the new out of state address(s) in MEDES.

TO CHANGE AN ADDRESS ON AN APPLICATION CASE
Change Person Page Evidence

- Navigate to the Person page > Evidence sub-tab > Evidence folder > Address evidence
- Click the List Actions
menu
- Select Edit
NOTE: DO NOT change any dates on the evidence

- Enter the new Address Details by typing over the existing address information
- Click Save
NOTE: Complete steps 1 – 5 for each person in the Household moving to the new address
Change Application Case Page Evidence

- Navigate to the Application Case page > Evidence sub-tab > Dashboard folder
- Click on Address hyperlink
- Toggle
open the Home Address evidence that needs to be edited
- Click the List Actions
menu
- Select Continue Editing
NOTE: DO NOT change any dates on the evidence

- Enter the new Address Details by typing over the existing address information
- Select all other household participants moving to the new address under the Apply Evidence to Other Case Participants section
- Click Save
- If the Validate Address pop-up window appears, select the appropriate address
- If the USPS recommended address lists a different county than the manually entered address, then select the manually entered address
- If the counties match, select the USPS recommended address
- If neither address displayed matches the new address entered, DO NOT select either address. Click Cancel at the bottom of the pop up window
- Click Save
NOTE: You must also edit the Mailing address as well as the Home address. If the participant does not report a different mailing address use the new home address and repeat steps 1-10, selecting Mailing from Type in the address details. The only exception(s) to this would be if the participant reports that they will continue to have their mail delivered to the previous address
NOTE: Ensure all Household members have DCN’s before completing the Eligibility Check

- Complete the eligibility check (if applicable)
- Make a comment on the HoH Person Page to include
- New address
- Any actions taken
NOTE: Address fields must have a minimum of three characters and cannot have more than 30. Use the second address field for apartment (apt) numbers, lot numbers, suites, floors, ect.
