Generate Request for Information for Application Case or Integrated Case

If additional evidence is required from the applicant in order to proceed with the application, the Eligibility Specialist generates a notification requesting additional information. The IM-31A Request for Additional Information form is printed and mailed from a central mailing location automatically. These notification forms are viewable in the HoH page > Client Contact sub-tab > Communications folder the day after generated.

An IM-31A Request for Additional Information is generated for each person with outstanding verifications. Instructions, specific outstanding items, and acceptable ways to verify are outlined on the form.

When generating an IM-31A, the evidence that verification is being requested for must remain pending for the system to correctly deny the application or start the adverse action to close coverage on an integrated case.

TO GENERATE A REQUEST FOR INFORMATION

1. Navigate the Application Case or Integrated Case> Evidence sub-tab > Verifications folder

NOTE: Ensure you have verified all evidence you have verification for before generating the IM31A Request for Information forms.

1. Click Send notice for outstanding…Wizard
2. Select the required notices

NOTE: If child support cooperation is required, also select the CS-201. See Cooperation in Pursuit of Medical Support for more information

1. Click Next
2. Type any additional information needed to add to the notice in the free form text box.
3. Click Next
4. Review and confirm information
  1. If you need to modify any of the information, click the Change hyperlink next to the appropriate notice
  2. Click Remove hyperlink to remove a notice
5. Click Next
6. Click Ok in the Notice Sent Verification confirmation
7. Make a comment on the HoH Person Page to include:
  1. The IM31A was sent
  2. What items verification is needed for
8. Review Notice Queue to make sure the form generated
