REQUIREMENT:
Effective August 28, 2015, all applicants for Temporary Assistance must receive a standardized orientation that provides information about the program rules and requirements. Applicants and second parents must sign an attestation that they have received this orientation. The attestation is contained in the IM-1TA application, or is available as a separate form when the IM-1TA application is not used. Compliance is captured on the TA Work Requirement (FM8B/COMPACT) screen. FAMIS will not allow processing of the application without compliance.
PROCEDURE:
This requirement may be met in various ways:
- All applicants should view the online video at application, including when at an FSD location, at the above link. The IM-1TA application becomes available at the conclusion of the orientation video. Applicants may complete the application online and print it, or may print and complete it by hand, then submit it to FSD.
- When viewing the orientation video is not possible, review and provide a copy of the print version of the orientation to the applicant(s), and obtain signature(s) on the IM-3 Orientation to attest to their having received the information. Print versions are available in English and Spanish.
- When necessary, applicants may listen to the orientation while on the phone with FSD staff, who may either play the audio recording or read the print version aloud. The applicant(s) must then sign the IM-3 Orientation to attest to having received the information.
When necessary, generate and edit the FA325 Request for Information following instructions contained in the memo cited above to request the applicant(s) complete the IM-3 Orientation (also available via this memo).
VERIFICATION:
Section 2 of the IM-1TA Application for Temporary Assistance captures applicants’ signatures attesting to having viewed the standardized orientation. Applicants who do not obtain the IM-1TA online or apply by other means must receive orientation and sign the IM-3 Orientation.
Applicant compliance is captured on the TA Work Requirement (FM8B/COMPACT) screen by entries in the Orientation/PRP field. (See Personal Responsibility Plan/PRP in this guide.) Compliance with both the Orientation and PRP requirement is captured in this field with the following codes:
- BC – Both Orientation and PRP Complete
- BI – Both Orientation and PRP Incomplete
- OC – Orientation Complete
- OI – Orientation incomplete
- PI – PRP Incomplete
